Actually, Yennifer is a hunchback. Was born as one - you learn this in the books. And she looks as she looks because, well, it's magic.

Actually most of sorceresses were unwanted girls most often ugly ones.

When they are past their first magical school initiation magic comes into play and they basically do plastic surgery on them. Yenefer was like that. Probably Triss too.

Also they are really old. Most of them are near century old (yennefer). Some of them like Terranova (wizard) had something like 300-400 years.

They are constantly maintaining some form of illusion magic and Geralt is, in fact, boning trolls during the entire Witcher series. Lulz.

If anyone gained actual magic powers, the first thing they'd do with it is make themselves look good. Fat chicks are lazy and weak-willed, they'd never be able to learn magic anyway.

What? Keira Metz looks like pretty much half the women on the streets every time I go outside. A cleanly dressed and made up career woman in her mid/late-thirties.

And yeah, you'd think that this would be a staple in any fantasy setting: if you know magic at all why wouldn't you do invasive cosmetic surgery on yourself?

Which one is it?
In the books, Geralt realises she's a hunchback when he feels her shoulders and notices they're slightly uneven, which she says was the result of her removing it via healing magic. So yeah, it's probably permanent.

Yeah, there's actually a sorceress in the book who's using an illusion spell to hide her deformed face (a missing jaw, as far as I remember), and as a result she looks kind of weird and is unable to talk. In the game Philippa is trying to use magic to create a new pair of eyes, which is said to be extremely hard but nonetheless possible. Magic plastic surgery should be a piece of cake in comparison.

This is how sorceresses look in actuality, without the magic:
The crones are not really human, though, as they're probably more ancient than elves. They use polymorphism, so their true form might be pretty much anything. Sorceresses look like regular people, although they're usually not that attractive by default as hot girls tend to get married instead of spending years and years studying magic.

It's permanent. Magic in the world of the Witcher is the equivalent of today's advanced medicine. Basic healing is done with the assistance of clerical magic.

It's not that hard to believe. If there are spells that are capable of compressing people to the size of dolls, or physically transforming them into monsters, then ones which heal physical deformities aren't that much of a logical leap.

The magic is permanent. Ugly daughters from noble families often are sent to train as sorceresses because it's hard to marry them off. They are made attractive because of the "prestige" of sorceresses. Sorcerers also change their looks but instead of looking like 20 year old super models they usually look around 40 but with an attractive "elegant" look.

Both IIRC, apart from Geralt boning trolls.

Depending on extent of ugliness/deformities and magical skill you had permanent physical changes (most of the time, actually since regenerative magic in Witcher generally can do this kind of shit) and illusions (usually when proper fix was impossible for some reason - like with Lydia van Bredevoort who effectively lost lower half of her face in a magical accident that somehow made her face impossible to reconstruct).

Sorcerers also usually don't come with the self-esteem issues that sorceresses have, due to aristocratic families usually being much more willing to send their sons off to learn sorcery than daughters (as daughters are far more useful for political marriages, plus sons are considered more likely to maintain their familial identity than daughters) and thus aren't 'expected' to be good looking as part of their job. Sorceresses get the double whammy of being essentially abandoned by their families due to their ugliness making them 'useless', and are then expected to radically change their appearance because it's 'expected' of them as part of their job.
In their first story together, Geralt's actually mildly freaked out by Yennefer because his enhanced senses make him pick up on all the aspects of her body that've been altered by magic: eyes that are slightly too perfect, shoulders that aren't quite evenly set, hips that are a bit too well-curved, etc. Leads to what Geralt calls "Beautiful women with the spiteful eyes of ugly girls".
