I've been recycling through same names for decades, don't care if they'd fit setting much. Pickinpetals and lilpai for male and female are my favourites currently.

If I'm larping a character, I try to give my toon a name that's fitting to the scenario or I make use of my vast fundus of pnp-names I used to give my characters. But that's actually rarely the case.
Most of the time I give the char my real life nickname or, if I'm playing a female: Lara Croft.
In party-based games I name the characters after me and my friends. I also used to utilize the name of my current girlfriend. But that led to abandoned saves one time too often after we broke up.

Usually after characters from various books/movies I like or from friends/family:

Eisenhorn - from 40K novels, he's typically the party leader
Knucklehead - Military slang for dumb ass, sometime party leader but almost always a melee class
Talos - from 40K novels, melee class
Renevent - made up name usually mage type character
Bender - robot characters
etc

Depends on the game.

If the protagonist is omnipotent, like in a strategy game, I just name them Amn. Where I'm playing a male often I just go with Doug because that is my real name. For females I alternate between Selena and Tamara.

I usually re-use character names, specifically from games like IWD where you could name the whole party - so there my lawful good paladin was named Necro Butcher, and whenever I make a similar character, I re-use the name. Also I often use my wife's middle name for the main character or games with a single character to name (not Necro Butcher).

So, I've been playing some ol' Might&Magic VI, and God, those fucking caverns never ends. My woman is making a dinner and cleaning sth behind me. She asked me for a hundred time why do I play those ugly, stupid games. I replied that M&M is not stupid and ugly - it's a piece of art. And in 1996 it was probably the most beautiful PC game. And I have started showing her my game. Screenshot included:

Suddenly she saw the names of my party which are (rough translation from Polish):
-Holy Dick
-Fag
-Forest woman (don't ask)
-Fucker
And she went crazy. She just could't believe that I have named my characters so vulgarly. She asked why and my response that I have done it for fun and to better remember who is who in my party only made it worse.
Now she is mad at me.

You know, this would be easier to tolerate if you actually were 13 years old.

If I cannot come up with anything I just steal names from other games, Valkyrie Profile had a lot of characters, for example.

I use a rotation of names for single character games and stupid puns off the top of my head for party based stuff (Florance the dual wielding sword user is the only one I still remember).

Most games, I use fantasy sounding names just to name the characters and get it over with, or pull out an old standard. However, there is something about Ultima 7 and U7 Serpent Isle that turn me into an idiot when I name my avatar...

Sometimes it takes me hours to name my party characters.

I must LARP them, identify with them, and give them life. Otherwise it doesn't feel like I'm playing a role-playing game.

In Wasteland 2 for instance I had a Sniper named 'Denver' who looked like a mountain man who had experience hunting with a rifle (very common in Colorado), had outdoor skills, the big utility backpack with a shovel, and everything else that fit that persona right down to the base stats.
