You can't upload a custom avatar unless you're a developer. So yeah, you have to choose one from the gallery.

If it's from a codex-related game you can make a package of avatars from the same game and ask to have it added to the gallery.

With the arrival of Thursday a great many traditional and established Codex-avatars were replaced by Age of Decadence rape-faces. I hope VD is proud of himself.
