To me the worst bane in MM2 was the Crazed Natives. I really hated those guys.

Also, not necessarily an enemy, but the main character in Mask of the Betrayer can do some ridiculously evil shit:
consume humanoids, betray and consume your companions to get their essence, skin alive a werewolf, torture, burn and consume spirits of the forest, send young barbarians, lost family and a teenage girl to be eaten by mountain cannibals, consume Myrkul (!), murder whole family of half-celestials.

think they were bad, try fighting Nessie.

The most evil enemies in a game are the developers. Always, without exception. If a developer cares in any way about his work, consider: You've got a guy who's making the game he always wanted to play, that he'll never get to play, at least not in the same way a player ever could. He's going to take it out on you.

Those first few original DROD versions didn't have the clock on the screen, you had to press Enter to see how many turns you had left before the Roach Mothers or Tar would spawn.

Yeah, I remember that. I just...must be reading wrong what version that's from.

I went and checked, and that's from the Architect's Edition, a.k.a. Mascochist's Guide to Bliss Edition.

In Darkness Over Daggerford - a module for NWN - you can use the blood of murdered children in dark magick potions to empower yourself. So the evilest enemy in any RPG is... yourself.

Warden Krieg from Grim Dawn. He goes from being pretty fucking evil in the base game to fucking ridiculously evil when you meet him again in AoM.

Stasis features several NPCs which are beyond fucked up.

The father on both LISA games ranks up there as well.
