I don't think Divinity 2 qualifies, it's not really open world and there's not all that much to do if I remember correctly.
Well, it's 3 open areas gated together. There are hidden quests, riddles, etc. to find.

Pretty terrible combat system, though. Quests/exploration kept me going.

properly modded new vegas or one of the TES games (preferably morrowind) i guess, everything else was mentioned already

there's also drox operative, but it's in space, doesn't have any dungeons and overall feels like a cheap and boring version of space rangers with RT combat.

Two Worlds.

I've noticed that some people enjoy it a lot, some hate it. I hated it at first but it grew on me fast after I learned how to dodge. The combat is nothing to write home about but the whole game just clicks and is better than the sum of its parts.

Combat's weird in Two Worlds with all that fancy-shmancy flourishing and twirling.
