Odd question but hear me out.

I want to motivate my players to have sex in my game. They should want to start relationships, get married, see prostitutes, etc.

Most games give you bonuses for seeing hookers that are totally unrealistic. Like in Pillars of eternity, spending a night with a prostitute gives Stealth, Lore, Mechanics, Survival bonuses. WTF????

MORE IMPORTANTLY, I don't want sex to just be a bonus, like a little cherry on the top. It needs to be a need.

This means that the longer you go without sex, something is being negatively affected. Any clue how to implemented? It has to be realistic.

The next adventure, or episode of the current adventure, in that world will be based on one of the children they sire/bear and they will be able to pick from the descendants that get rolled?

Well, you know how when you rest in an inn, this is how you restore your hitpoints? If you want your adventurers to behave like the adventurers in media, just make this part of the hitpoint restoration process. I mean, why ARE these people getting hammered in taverns and banging hookers? Obviously, because it restores their hitpoints. Allow characters to choose from a variety of hitpoint-restoring activities appropriate to their background and class, and for some characters, this is going to be "hookers and booze".

I recommend reading the classic Dragon Magazine article "Too much loot in your campaign? D&D OPTION: ORGIES, INC." by Jon Pickens, which originally appeared in Dragon Magazine #10 (October, 1977). Pickens proposed awarding experience points for treasure when such treasure is spent rather than obtained, citing none other than Fritz Leiber's sword-and-sorcery duo Fafhrd and the Gray Mouser as examples. He also made five suggestions as to common ways of spending treasure, the last of which was "Orgies. Fighting Men (excluding Rangers and Paladins), Bards, Thieves, and all Chaotics (excluding Monks). Lusty indulgence in wine, women, and song. Maximum expenditure is 500 GP per level per night (250 GP if recuperating and under 50%). A player may orgy continuously as many days as he has constitution points, but then must rest for as many days as he orgied. (For effects on Psionic Powers, see Appendix II.)"

TBH, I don't think there's a market for non-porn games with sexual satisfaction as a central mechanic, and if you are making a porn game, then just give the player a depiction of the act and be done with it.

That said, maybe you can tie it to a morale or sanity resource, which is itself central to the game, and affects your characters in battle.

Realistic you say? That's complicated because everybody reacts differently to sex. From addicts to asexuals.

From a purely physiological standpoint, I'd say having sex gives a short term penalty to physical stats, like athletes that aren't encouraged to have it during competition.
For long lasting effects, I'd say having sex basically makes you more balanced, more healthy mentally. But if you go without it for a long time, it stops having any impact. So penalties and bonuses that are capped. A stat at 10 could gradually decrease to 8 without sex or increase to 12 with regular sex.

Now if you want to have fun, you should also have classes like priests or monks that get bonuses for not having sex. :)

Priests get bonuses proportional to how young their sex partner was from a base of 18 years:

Priest sleeps with 18 year old girl: 18 - 18 = +0 bonus
Priest sleeps with 13 year old boy: 18 - 13 = +5 bonus to all stats.

Have an "incel bar" that gradually fills up as the game progresses.

The more the bar fills up, the more negative consequences the player experiences.

Wrong approach. This is why all porn games are disappointing garbage. You go through normal combat and all that, then get a static sex scene with the defeated enemy without any player input. Lame.

Do it like Lilith's Throne and make sex into an actual gameplay mechanic if you want to do a proper porn game.

I agree with JarlFrank . Sex games that simply require you to fulfill some arbitrary sexual satisfaction bar tend to make gameplay halting and stuttering in nature. It's better to integrate sex into the game as a variety of different mechanics. I actually am trying to do something like with my own game, though I must admit, this is a difficult task. It hasn't really been pulled off well before.

Here's my solution that I'm going to implement into my game: Stress

Stress increases from combat, work, companions dying etc.
Stress reduces all your charisma actions proportionately, and extremely high stress reduces your combat efficiency by a small %
To reduce stress, you can get drunk at an inn (low stress decrease), get drunk at a feast (medium stress decrease), or have sex (100% stress decrease)

If a player has a girlfriend/wife in his party, his stress is automatically decreased by 100% each time the party rests for the night :smug:

3D renders are pretty easy to do nowadays if you look a bit.

The problem with porn games are that they are fucking boring. Add some eroticism, add some teasing, add some fun and games, add some mind breaking with drugs and magic.
It's always the same boring vanilla relationship progression with pointless grinding.

If you want things to be much more than just a porn game then you need descendants like in Crusader Kings with complex genetics, breeding and inheritance. Maybe with even dark rituals for even more overpowered.
If you aren't banging super beings to get a ultimate God as a Son that you will eventually play as you aren't even trying.

For a Player the ultimate Need is always Progression towards even more Power. Whether you are bangining princesses to get the kingdom, or banging devils to get a demon king.
