It seems the most challenging games are either recent games (that I haven't played myself) or old games (1994 and older). Not much from the CRPG Renaissance era.

If you count it as an RPG (it does have some pretty extensive character and squad customization), give Mordheim: City of the Damned a try. The game has a forced iron man mechanic around which it is balanced, plus a combat system that emphasizes risk management. To be fair, it does get easier once you figure out the AI and the mechanics, but isn't that the case with a lot of tactical RPGs? I also second the Elminage Gothic recommendation, it's probably the most challenging of the modern Wizardry-like blobbers.
