Fuck flashy intros and explanation. There are over 20,000 New Vegas mods on Nexus Mods, and out of the 100 most endorsement mods, I will list out the ones which are genuinely good, a.k.a. Sigourn's Top 100 Picks. Note that this isn't a list of "must haves", just a list of really good mods. It's up to you to decide which ones tickle your fancy, the point is that you won't have to sort through garbage to get to them. Before each mod I list its current position on the Nexus' "most endorsed" list. It helps give you an idea of how much shit we are skipping past.

3. The Mod Configuration Menu
  • Essential? Absolutely, if you are serious about installing mods. Many popular mods require this to tweak certain aspects in-game.
  • No downside to having it installed either.
7. NVAC - New Vegas Anti Crash
  • Essential.
  • No one should ever go without installing NVAC. Helps prevent New Vegas' famous crashes.
9. New Vegas Stutter Remover
  • Essential.
  • Considerably reduces microstutter.
  • Windows 10 users should use New Vegas Tick Fix instead.
20. New Vegas - Enhanced Camera
  • Goes well in tandem with body replacers and while playing as a female so you can check out your fantastic tits and live out your HRT fantasy.
  • The .ini file provided by the mod lets you tweak certain aspects of the mod, iincluding its most annoying features.
  • Overall increases immersion as it lets you see your body in first person.
25. The Weapon Mod Menu
  • Weapon modification done quick and easy through a very cool menu.
27. UIO - User Interface Organizer
  • An essential utility if you install interface mods, one of which is, in my opinion, an absolute must because of how much it improves on the vanilla UI while keeping its aesthetic intact.
30. Yukichigai Unofficial Patch - YUP
  • Essential.
  • Like Morrowind, New Vegas also has its handful of unofficial patches. This is the one you want, as it stays away from unnecessary bullshit. Some people criticize it, but the alternative is to "install the fixes you need" as if New Vegas wasn't one of the buggiest games ever made (and by extension you needed to install over 100 individual fixes).
32. Interior Lighting Overhaul
  • As a rule of thumb I stay away from visual mods unless I have a very good reason to install them, which isn't "it's low res". Most of the time it is because of consistency. Other times is because they genuinely add to the game. This is how I feel about lighting mods: there's a reason why photography is one of the most praised aspects on good films. Interior Lighting Overhaul dramatically alters the lighting of interiors. There are minor incompatibility issues which are usually fixed by placing this mod near the bottom of your load order (that is, loading after other mods).
34. Enhanced Blood Textures for NV v2_22c
  • The other exception I have with visual mods is "effects". I can deal with low resolution textures, but not with low resolution or poor quality effects, particularly in a game where the graphics are of "considerable" quality (we are not talking about a 90s cRPG here). Vanilla blood looks terrible. This mod makes it look much better, and thus adds some much needed gore into post-apocalyptic gunfights.
37. FNVEdit
  • This is not a mod but an utility. Anyone who mods New Vegas and wants to keep conflicts at bay needs to get used to this utility ASAP. It makes solving conflicts a very simple task, and I wish Morrowind had the same .esp structure as New Vegas does (alongside a similar utility) because of just how good this tool is.
40. FNV Realistic Wasteland Lighting
  • My favorite lighting mod, by far. RWL gets rid of the game's orange tint, replacing it for fairly realistic lighting, appropiate to the Mojave desert. It's also a very lightweight mod compared to the vastly more popular Nevada Skies (which I don't recommend).
41. One HUD - oHUD
  • Very neat mod that lets you move around and hide existing UI elements.
  • Also adds additional information for hardcore needs, combat stats, etc.
43. JIP LN NVSE Plugin
  • In addition to NVSE (not hosted on Nexus Mods), JIP LN is another plugin that extends New Vegas' scripting capabilities, making the existence of many great mods possible.
  • This plugin also comes with an .ini that lets you enable different features, such as Fallout 3 repairing (which limits how much you can repair a weapon based on your Repair skill), and helmets being the sole equipment that accounts for head protection. In vanilla, equipping any armor gave you head protection, so what should be lethal headshots were nothing but grazes on NPCs' heads.
50. Populated Casinos
  • Because of console limitations, The Strip hosts some of the emptiest casinos on the face of the Earth. This mod corrects this by adding many, many NPCs inside them.
57. FNV 4GB Patcher
  • Makes Fallout: New Vegas 4GB aware and instantly loads NVSE if present.
149. Collision Meshes
  • In most games, collision meshes can suck and you won't really care. When it comes to a shooter, you want air to be actual air and not just "invisible steel".
  • This mod also comes with quite a few fixes.
153. DYNAVISION 3 - Total Visual Enhancement
  • Neat mod that adds plenty of visual overlays, including grain and lighting/color filters. Using Realistic Wasteland Lighting, however, I found this mod somewhat pointless as I could find no settings combination that improved on it.
164. Improved LOD Noise Texture
  • The vanilla LOD noise is goddamn awful. This is a massive improvement on it.
193. IMPACT
  • Massive improvement of ballistics-related effects. Unlike the author's EVE (for energy weapons), this mod doesn't modify anything other than these special effects.
  • The much less endorsed IMPACT - Compatibility Edition (JIP LN) (DLC - TTW - All Mods) should be used alongside IMPACT's assets (disable the .esps) to maintain compatibility with other mods.
215. Follower Home Marker
  • Allows you to set your home marker that fired companions can return to, instead of only the Lucky 38. The home location is used/shared by all the vanilla followers, so you only have to set it with one of them and it will affect them all.
227. Alternative Repairing
  • In case you haven't been paying attention, I'll say it now: this is the first gameplay mod in this entire fucking list. On position 227, of all. Alternative Repairing is a delightfully complicated revamp of the game's otherwise LAME repairing system. Best part of all? It is surprisingly compatible with many, many mods. All mods I recommend, actually. Give it a try if it sounds interesting to you.
258. Missing Ammo Recipes
  • Another gameplay mod? And it only took us 31 shitty mods to get to it! The cool thing about Missing Ammo Recipes is that it blends in seamlessly with the vanilla game. It's such a neat mod and so simple that someone who has never played vanilla New Vegas would have an impossible time realizing this is a mod. Though not a patch, I do believe this is a must have, if only because of how inoffensive yet awesome this mod is.
283. Eyelashes New Vegas
  • Simple beautifying mod that adds eyelashes to the game's characters. Believe it or not, this goes a long way towards making NPCs seem more human and less... zombie.
299. Ghouls Hires retexture
  • AMAZING retexture of ghoul NPCs. Compatible with body replacers, a major plus given how nice this retexture is.
We have 30 mods so far (not counting FNVEdit here). Those are rookie numbers, especially considering 9 of them are related to body replacers/character visuals. Let's keep on browsing.

304. OneTweak for FNV

  • This mod adds a borderless windowed mode, Alt+TAB functionality, and the double cursor fix.
324. Gun Follows Crosshairs in First Person
  • Does what it says on the box: your gun will always follow its crosshairs in first person.
325. Vault 22 Flora Overhaul
  • I'm listing this mod here for a simple reason: it is a really cool addition to the game, expanding one of my favorite New Vegas Vaults. But it comes at the cost of performance. In my personal experience, a very important cost that turned this area into an insufferable mess the last time I went through it. My rig was outdated the moment I got it, which was about the time this mod was released. Nowadays, and assuming you've got a good rig, I would be able to recommend this mod to you.
340. Tutorial Killer
  • Even though I find the game's tutorial messages annoying, I never thought they were SO annoying they required a mod to get rid of them. That said, this IS a functional mod that does what it achieves, without dumbing down or simplifying the game in any way or form, which is why I'm listing it here (as opposed to the many "unlimited ammo", "essential companions" and so on shit I've been skipping by).
375. JIP Improved Recipe Menu
  • Many quality of life improvements to the game's rather wonky crafting menu.
392. No Auto Aim
  • A common criticism of New Vegas, even here on the Codex, is how the game uses dicerolls to determine your shots landing on the target. This gets rid of that feature.
393. FOV Slider
  • Another common criticism of New Vegas is that, in spite of being a "PC" game, it lacks an FOV slider. This adds one, and lets you tweak the different FOVs found in the game (such as the world FOV, the Pip-Boy FOV, and your gun FOV).
411. Uncut Wasteland
  • New Vegas is a game noted for its rushed development, which led to plenty of cut content during production. But what not many people know, particularly those who never played the game on PC when it was released, is that the official patching process deliberately cut down on the amount of clutter and NPCs you could find in-game to make the game run better on consoles. What this mod does, unlike the different "cut content" mods found on the Nexus, is restore the cut content due to PATCHING after the game's official release. In practice, this means you won't run into anything "strange", any uncut content that feels unfinished (because it was cut for a reason, ya?).
  • A must have IMO. I recommend the Uncut Wasteland plus NPCs release.
419. Unofficial Patch Plus
  • Unofficial Patch Plus is an additional collection of fixes for Yukichigai Unofficial Patch. I have a few things to say about this mod. The first is that many gameplay tweaks are included in an additional "Addendum" file. The second is that there's an alternative to this mod, called Unofficial Patch NVSE which, for all intents and purposes, would be the "true" Unofficial Patch add-on to YUP, since it stays away from gameplay tweaks, period.
  • Because I agree with pretty much every gameplay tweak included in UPP, it is what I use. That said, pretty much all its gameplay tweaks can be found as separate mods on the Nexus, should you want to install them later.
422. Ragdolls
  • Improves ragdoll behaviour for all NPC/Creatures in the game. Pretty cool mod, not going to lie.
437. FeralGhouls Hires retexture
  • This is the sister mod to Ghouls Hires retexture, retexturing feral ghouls (which use different meshes and textures).
461. Loot Menu for FNV
  • I DON'T LIKE THIS MOD, but that said it works as advertised and I know many people would like it. I appreciate the real-time looting which can make battles just a bit more exciting, and that's always welcomed. But I find it a bit weird and that's why I don't use it.
477. FNVLODGen
  • This is another utility that does nothing on its own. However, you can use it to generate even more detail to the game's LOD. Bear in mind this is NOTHING like Morrowind's far, far superior MGE XE. Prepare to be disappointed if you go in expecting MGE XE-like levels of detail.
496. Improved Automatic Ironsight Animations
  • The vanilla automatic ironsight animations suck. What this mod does is remove the fake recoil (achieved by camera movement) and replacing the animations to ones with actual recoil.

It was originally going to be much, MUCH bigger, explaining why most of the mods found in the top 100 are garbage, but then I realize people would just TL;DR. Yep, zMerge is pretty cool, this list is by no means exhaustive, just the good stuff you can find by browsing the first 25 pages. It's a shame that it's cluttered with crap, though. I can understand it with the most popular mods being released in New Vegas' infancy (2010, 2011), but it's still annoying. To bring Morrowind into the comparison is not really fair as the game has a much smaller playerbase nowadays and Nexus misses the first six or so years of Morrowind mods.

Yeah, they aren't garbage, they are just outdated. The problem is that it takes a while for the newcomers to understand this and until then you will be dealing with a lot of "can I use this with Project Nevada?".

Okay, shitposting aside, decent list. Checked out the Yukichigai Unofficial Patch readme and the modder was very thorough yet I didn't notice a fix for the Lonesome Road allegiance shenanigans.

Would also like to recommend Clarity, it's a simple mod to remove the horrible orange tint and keep everything else vanilla. Works in first and third person view.

The TL;DR is:
  • Some of them are buggy.
  • Some of them are outdated.
  • Some of them are waifu shit of the worst kind. I even removed the waifu mods I had listed because I knew some people would discredit the entire post because of them.
  • Some of them are absurdly unpolished.
  • Some of them are known to cause crashes.
  • Some of them have massive compatibility issues.
  • Some of them are cheat mods.
Say, let's take the 20 most endorsed mods, just to make it quick:

PROJECT NEVADA
  • Buggy.
  • Outdated.
  • Unpolished in a few modules.
  • Compatibility issues with major rebalances.
FALLOUT MOD MANAGER
  • Outdated. Completely superseded by Mod Organizer 2.
NMC'S TEXTURE PACK
  • Isn't faithful to the vanilla artstyle.
  • Other than that I have no issues with it. I simply don't use texture mods.
NEVADA SKIES
  • Buggy.
  • Bloated.
  • Lack MCM support.
  • I believe RWL looks much better.
EVE - ESSENTIAL VISUAL EFFECTS
  • Massive compatibiltiy issues.
  • Makes unnecessary visual changes to weapons (if you are only interested in the special effects).
WEAPON MODS EXPANDED - WMX
  • Massive compatibility issues.
  • Possibly unbalanced.
  • Not really necessary, the negatives outweight the positives.
WEAPON RETEXTURE PROJECT - WRP
  • Compatibility issues.
  • The author shat on the vanilla textures to the point of removing the Virgin of Guadalupe from Maria, the snakesin grip from A Light Shining in the Darkness, and the broken down looks of the Survivalist's Rifle.
FALLOUT CHARACTER OVERHAUL
  • Everyone looks like a zombie.
  • Massive compatibility issues.
  • Some features are buggy. The glowing ghouls add-on is known to cause massive lag.
BEWARE OF GIRL TYPE 3
  • DIMONIZED is better for Type 3 meshes; zzjay's textures are better; the recently released TYPE 4 is of a much higher quality, sadly some outfits stray away from vanilla.
NEW VEGAS BOUNTIES I
  • Bad voice acting, bad quest design, uninstalling quest mods mid-save is a pain in the ass.
MORE PERKS
  • Unbalanced and unpolished.
WILLOW
  • There's a Tumblr meme waiting to be made here. The truth is these companions suck.
PERK EVERY LEVEL
  • Completely unnecessary.
WEAPON ANIMATION REPLACER - WAR
  • I was going to leave this one in. I personally don't use it, but it's not a bad mod at all.
WASTELAND FLORA OVERHAUL
  • This one is a perfectly acceptable flora overhaul if you use the "dead" (a.k.a. lore-friendly by New Vegas standards) version.
PROJECT NEVADA PATCHES
  • Unnecessary since Project Nevada is bad.

All of this said, when it comes to mods I prefer to keep things
  1. Stable.
  2. Simple. Reasonably so at least, usually the most complex mod is the overhaul followed by mods that expand on entire features (such as Alternative Repairing). This helps avoid compatibility issues, too.
  3. Necessary, or near necessary.
There are plenty more mods coming if people show interest in such a thread.

No joke I'm surprised Bouncing Natural Breasts is such a popular mod when it demands far more work from the average Nexus user than installing a simple patch that people most likely don't install.

Interesting. I have used some of these, but now that I consider them more carefully, I find myself agreeing with you. EVE was an utter nightmare to get working. New Vegas Bounties was interesting the first time, but on repeat playthroughs I found myself questioning why the fuck I'd ever liked it. I do like most of the NMC texture pack, though.

If you feel like going over more of the top 250, I would definitely be interested. No pressure, though. I won't be revisiting FONV for a long time.

283. Eyelashes New Vegas
and this is just pathetic

Populated Casinos is a pretty good mod, actually. That's because it does nothing but add more fluff NPCs to casinos, NPCs which are already present in the game so no harm done. And it runs just fine on my computer, which isn't anywhere near top of the line.
And the eyelashes mod is great, don't shit on it until you've tried it. Anything that improves on the vanilla NPCs is a huge bonus, especially when compatibility issues are kept to an absolute minimum.

No sexout? Disregard.
  • Compatibility issues.
  • Buggy.
  • Lots of scripts running.
  • Animations are wonky and are tailored to the inferior Breeze's Males.
  • Characters ragdolling after sex is immersion breaking.
  • Sex lines lack voice acting.
  • Makes you install a shitload of terrible assets just to install the mods you want to use, and which in turn make use of these terrible assets just because it is an obligatory download.
I'm preparing Round 2 as we speak.

Project Nevada might not be perfect but what the hell are you doing playing without any kind of substitute for it? What are you using to escape the bullshit vanilla bullet sponge combat? We've got great looking eyelashes but we still have to unload 50 shotgun blasts into someone's head at point blank to do any damage.

After the resounding success of the first 500 mods, here I bring you the best mods from the Nexus' 501-1000 mods endorsed mods.

501. NVCE - New Vegas Character Expansions
  • Essential mod that depotatoes many an NPC and makes a major improvement to Joshua Graham's appeareance.
  • This mod was, in my opinion, rendered obsolete with the release of Character Expansions Revised. The original NVCE made many unnecessary changes to NPCs in the name of "consistency", like altering Karma levels, entire inventories, and more. Character Expansions Revised keeps it simple by simply altering the appearance of NPCs. This way making compatibility patches is a very easy task.

518. ADAM Reborn
  • The truth is, while this mod is a visual retexture of NCR equipment, this is not why I suggest using it. Rather, it is the beautiful recreation of the NCR Ranger Combat Helmet (plus the improved coat meshes) as seen on the game's cover art that makes this a must have IMO.
  • Select the Custom Install.
  • Activate Exeter's Improved Coats only (coats will drop down as opposed to awkwardly floating around your character).
  • Select the Original Helmet.
  • Afterwards, disable all .esps. You don't need them.
531. JSawyer Ultimate Edition
  • A revamp of Josh Sawyer's own jsawyer.esp, JSawyer Ultimate Edition aims for compatibility and to improve on the original mod by the balance man.
  • Features compatibility patches for some very good gameplay mods.
  • Comes with a sprinting feature and other MCM tweaks, such as Perks per level, health formula and carry weight, amongst others.
543. Mojave Music Radio - Extended
  • The only radio mod you'll ever need. There are plenty radio mods for New Vegas, but this is THE ONLY ONE that stays true to the western theme of the vanilla radio.
  • My personal favorite: Gene Austin's Lonesome Road.
562. Pip-Boy 2500A - The Arm Mounted Pipboy 2500
  • Some people are desperate for Pip-Boy replacers. Of the ones I've tried, this is one of the best, but not my personal preference.
  • It is designed to emulate the Pip-Boy aesthetic of Fallout and Fallout 2.
567. Precision Collision - Clutter NV
  • Makes massive improvement to clutter collisions.
  • People have reported some performance drop because of the more detailed collisions. Moreover, this is anything but a must have unless you like to spend time decorating your home base. Even so, the game's engine is shit at decoration (unlike Morrowind, since it lacks physics), so I would skip this altogether.
  • I only list this mod because it is one of the most popular "collision" mods out there.
598. Vanilla Hair - No Shine
  • The vanilla hairs shine like there's no tomorrow. Attribute it to greasiness if you want, but this mod gets rid of it.
599. More Realistic Aiming
  • Gameplay mod that makes it so that different stats influence your character's aiming, such as your hardcore needs. It's a pretty good mod and a must if you dislike the game's clunky shooter mechanics.
650. Convenient Fast Travel Markers
  • I've played New Vegas without fast travelling in the past. It was a terrible experience. The vanilla travel markers aren't particularly good for the most common areas you will visit in the game, such as the Strip or the Fort. This mod adds many more fast travel markers to make getting around the Mojave a simple and pleasant experience.
662. Lonesome Road True Faction Allegiance
  • The Courier Duster you get at the end of Lonesome Road is one of the coolest pieces of equipment you get in the game. It's also very wonky at determining your faction allegiance and thus the symbol on its back. This mod corrects that.
668. EXE - Effect teXtures Enhanced
  • This mod supersedes EVE when it comes to visual effects and visual effects only. A perfect mod for those who wanted EVE but didn't want to deal with the compatibility issues its energy weapon edits had.
689. Enemy AI - Tactics - NPC Healing
  • Gameplay mod that makes fighting NPCs that more interesting by letting them heal in combat and use "tactics".
702. Clarity - An Orange Tint Remover

  • Yet another SFX mod: this makes the glass impact decals look far better.

  • Makes the path lighting of Old World Blues lit up at night.
826. Power Armour Scaling
  • Makes people in Power Armor look bigger. Honestly skip on this if you think "size" is the least of the issues NV Power Armor has.
848. Tweaked Standing Idle
  • Vanilla characters are always slouching and it makes them look like shit. This fixes that.
  • Install Tweaked Standing Idle by Documn Fix on top, as it irons out some issues the original mod had.
875. Better Pickup Prompt
  • Adds quality of life improvements to the pickup prompt, such as accurate weight and value when accounting for stacks, and whether picking up an item will encumber you.
891. B42 Weapon Inertia
  • Adds a very nice weighted feel to weapons as you no longer move them around as if they were made of cardboard. A visual mod that goes a long way towards making guns feel more believable.
897. Improved Transportalponder - Old World Blues
  • Quality of life modification for the Old World Blues Transportalponder. Teleport companions along with you, return to the last place you were at, and get a nice visual effect as a bonus.
898. Primm Reputation Restored
  • Restores the cut Primm Reputation.
902. Vanilla HUD Remastered (4K)
  • While I personally think the vanilla UI resolution is fine, some may disagree. This mod increases the resolution at the expense of modifying the UI's visual style.
  • Compatible with Vanilla UI Plus.
906. Bullet Impact Increased LOD
  • Increased the length at which you can see bullet impacts and blood.
910. HD Hi-Res World View Weapon Textures
  • Makes weapon world models use the same textures as weapons in first person, which are of higher quality.
935. Oven Cooking
  • Lets you use ovens as crafting tools, much like campfires.
  • Very nicely integrated into the game, as not every oven is functional and thus requires repairing, plus additional flamer fuel is required to get them working.
  • Handy when there are no campfires around.
948. Strip Wall Billboards
  • Restores the cut billboards found on the Strip walls.
  • Not a lot of variety, but certainly more appealing than the otherwise flat billboards.
958. Better Gas Leak Effect
  • Visual effect mod that improves on the gas leak effect.
  • The main reason you should install this mod is that it makes gas leaks far more noticeable. This is crucial to spotting gas traps.

Some good mods here... funny how other than a few tweaks and patches New Vegas is pretty much perfect as it is

Perfect? Its anything but. But i would still use just 2 or 3 mods for it, and those would be:
1. Slower backpedaling
2. Dark Nights
3. Trapponator 2000 or how was it called, for traps.

Everything else is unnecessary cluter that just prolongs this agonizing game.

Weak. Regarding the other two:
  • JSawyer Ultimate covers slower backpedaling.
  • Realistic Wasteland Lighting covers dark nights. Alternatively, the Interior Lighting Overhaul author released Nights are Darker - Ultimate Edition, which is a suitable replacement if you only care about dark nights, period.

It makes trap detection work in a sensible way.
I know what those other mods have, but you really need only parts of them. No any kind of "realistic" lighting, just dark nights.
And just backpedaling, without the JSawyer component.
