The original isometric Fallout games were really mediocre. We just had come out of a short CRPG decline period and Fallout alongside Diablo re-ignited the western CRPG efforts.
Fallout had a ton of faults but we forgave those because we liked the setting, the maturity, the c&c, and the fact that it wasn't fantasy. Looking back, it wasn't really that great, and there is a reason this style of game never caught on and clones of it never had any success.

There is no logical reason to urinate in the toilet instead of the sink.

There is. Urine can attract various microbes and pathogens and since the sink is used for stuff like washing our hands, brushing our teeth, washing our face, it is better to not urinate there, since this would make us need to disinfect the sink after each urination.
