The ability to potentially kill any NPC. No "is unconscious" BS by Bethesda etc.

Immersion in another world would be my number one answer.

If I am allowed a top three, I would add:

2. Exploration that has a reward attached, like unique weapons or story content.
3. Power fantasy of taking dudes out from the shadows and escaping without being seen.

Story. Everything else is meaningless unless I get a great storyline to keep me motivated.
