The Minsc joke probably caused more rage from the GamerGate camp overall. The Tranny was just icing on the cake, and taken alone probably wouldn't have caused much more than a shake of the head.
I don't think so. I mean, I'm all for GamerGate, and I still find the inclusion of a transexual in a world where their plight would be very easily solvable to be disrespectful to the source material, much more than a silly (unfunny) aGG joke.
So it alienates fans (and most of those are probably indifferent to GG) because of the following:
1) It's completely out of place in the setting. Like it was already mentioned, this could have been easily solved by making it into an interesting little side-quest: "Please bring me a girdle of femininity."
2) Dreadful writing. Even if you are a staunch believer in diversity, you can't deny that this was written amateurishly, and in-your-face.
3) You get no chance to react to it. It's either: "Oh, I see, how interesting.", or "Ok. Well, goodbye.", which is totally jarring, because you don't even get the chance to be a sarcastic idiot or evil character, something which you can do in most other situations.
