Also, not necessarily an enemy, but the main character in Mask of the Betrayer can do some ridiculously evil shit:
consume humanoids, betray and consume your companions to get their essence, skin alive a werewolf, torture, burn and consume spirits of the forest, send young barbarians, lost family and a teenage girl to be eaten by mountain cannibals, consume Myrkul (!), murder whole family of half-celestials.

Maybe not faced uber-directly, but yeah. Allied Mastercomputer was a freaking scary and evil thing. It murdered whole mankind except 5 people, just to make them suffer.

As for 'annoying' evil, anything in any game that stunlocks you until death when you make even the slightest mistake. I hate those fuckers.

The "lion" colossus in Shadow of the Colossus likes to do that if you fall into the sand area in front of his cave, and you probably will at least once since you have to fool him into falling there first and quickly follow him to jump on his back. He also scares away your horse so can't grab onto it to escape, instead you have to watch him pummel your flailing body for five minutes.

On that note, this fucker as well

His electric bolts are so fast and make such a huge splash that you can't dodge them on foot. So you have to keep riding the horse at all times, while you keep the turtle on sight and try to coax the thing into moving on top of one of the geysers, so the jet will hit its underside and raise one side of its body, so you can shoot an arrow at the bottom of its feet...that is, unless you got too far while dodging the bolts, which means it'll return to normal position and probably move away from the geyser.

This thing used to give mi nightmares. The Strogg from Q2 also deserve a mention, the thing that creeped me out the most was how they were made (that "stroggification" facility level).
