That's one of the reasons why point-and-shoot cameras double flash nowadays, to make pupils contract and prevent red-eye, which is the reflection of the flash off of your retinas. They also do this for other reasons including to focus properly, but that's complicated and a completely different story that changes depending on the brand/camera.
However, most professional cameras can preconfigure the flash level so that they only flash once, the better to capture natural photos. Judging by the era this was taken in and the quality of the photo and flash, it was clearly not a smartphone camera that took this photo. Hence, a professional camera likely taken by a professional who preconfigured the flash (hence disabling the preflash) like a professional. There is no reason to expect any unimpaired person in the same situation to have smaller pupils. QED.

As I read about it, the main problem is not the tweet in his case, but the sexual harrasment accusations which made him lose his job on several projects. So there were consequences for those tweets.
