Well, according to this thread on the Interplay forums, that creature is something called a Hunger from Morrowind. I haven't played the game, so I wouldn't know.

Sawyer is also infamous for confusing the fanboys,

I doubt that. It does resemble hunger's overall form as its a bipedal humanoid with claws and a tail, but this description relates to a LOT of bad things out there. I don't recall a hunger having pointy-looking things (horns, ears?) on its head, or an athletic chest.

Apparently, the image looks pretty close to the 3E Monster Manual description of the Osyltuh, some sort of Baatezu. Might be an indication of the sort of baddies you get to face off against.
