These games haven't really stood on their mechanics since vic 2 and are mostly larping simulators, they'd have 0 players if the board was set in any imaginary place.
Outside of paradox, most game larp is in fantasy settings. Mods like Anbennar and Elder Kings are popular too, so there's clearly an audience for a fantasy title.

The main obstacle for a paradox fantasy game is the effort and creativity required for a new setting. It might also need a niche their other games don't occupy, unless it's just a CK3 spinoff.
