It has lots of cool stuff in it. Like the way injuries are handled - you have bodyparts which have their own HP. It can go below 0, but then that bodypart is fucked. If it's torso, head or stomach you're KO when it passes KO level(dependant on your toughness). If it's limb it will be useless(forcing you to switch to 1h weapon or try to fight from the ground) if it passes negative threshold for given creature/race it falls off triggering massive bleeding(you can get robotic limb to replace it, but one faction will attack you on sight if you have anything robotic). Torso, head, stomach and blood going below that threshold cause death. Of course wounded bodyparts carry penalties like legs will reduce your running speed(causing you to limp) while torso will reduce your strength. You have to bandage the wounds(or else they'll degrade) and then they heal... slowly.
It sounds boring but it means that bigger fights end up with hordes of characters slowly dying after being KO'd and not healed, some of them that managed to regain consciousness but were bleeding out because of for instance cut off legs will crawl away from combat leaving trail of blood until they collapse completely, tons of limbs lying everywhere etc. etc.
Nowadays I don't give a fuck and play the game with 5x exp multiplier mod, fuck the early game grind.
