I replayed through KotC in 2020 and barely even remember him. You'd probably be better off checking the official forums for specific questions.

Steve gets a Kidney but I don't even get a tag.
He's one of the two Slaver Lieutenants in the Orc Stockade along with Kessa who is a mage. It's fine, I went through the dungeon and I got the opportunity to kill him at the end anyway.

Is the option "respawning enemies" handcrafted encounters or just some random stuff that happens sometimes

Just random groups of enemies spawning in dungeons. Nothing fancy.
