It's almost definitely a hologram.Since the protagonist's eye functions like a computer, the blur could be caused by him being shot at and just reboot the system, it doesn't really confirm anything.
They are finally being honest about it. Imagine how far this game is from an RPG if they think Witcher 3 is an RPG.
