If it's two consenting adults using protection, I don't think there's anything "wrong" going on. It's just that people that grow up like siblings (doesn't even have to be any genetic relation) normally develop a deep abhorrence to the thought, so it never really happens. It's more of a built-in mechanism rather than a true moral conundrum.
