Eh, I wouldn't say that POTD diff swings that much - there is some spike at the beginning, but around lvl 7-9 it flattens out and goes downhill from there, mostly due to spells you get around that time (confusion, devotions, some cipher ones etc etc). E.g. confusion and call to slumber trivializes all of the fights with ogre druid packs.
Beside WM dragon I can't recall any really tough/unfair fight tbh.
To be honest, I mostly avoid per rest spells much more than I should, unless it's a boss battle. I also tend to avoid campfires. So my play is a bit skewed. I had six campfires by the time I finished Durgan's Battery (two that I was carrying, and 4 more lying around inside the battery that I hadn't used). Even with the two rests I did, I was doing them because my party was dead. Aloth usually had half to 3/4 of his spells left.
I checked the flags, I had only upscaled White March, not Elmshore. So the Sky Dragon I was having difficulty with was actually just the normal PotD Sky Dragon. I thought I would give it another try, with a little more prep (setting up some consumables, swapping around a bit of the equipment), and she ended up doing while doing barely any damage at all to any of my characters? Everyone was still in the green, almost all the way up to the top of their health. I did a little prep and changed my tactics a little, but it didn't feel like that much of a difference for such a drastic change. One thing I noticed, though, was that the dragon kept targeting Eder, and never did a single breath attack, even when everyone was at a distance. Both of these things made the battle much easier, but I'd have to try the battle a few more times to tell if the different AI was just RNG luck or because of different equipment.
It's my second attempt at PoE (I gave up ~3/4 of the way through my first), and looking at my playthrough I started it almost 4 years ago. Maybe I'll finish it up next year if I have time.
