You need to differentiate between a deliberately racist work and one that has racism as a crucial element of the setting "in-world".

Both TES and Arcanum mentioned above are in the latter category.

A major element of all TES games is the social tensions around the races, above all between Men and Mer, that could be a cosmological conflict that goes back to the very origins of its world. Arcanum was created wanting to explore how the Victorian Era would be if there was a conflict between magic and technology in a generic fantasy world undergoing an Industrial Revolution, and again, race was a crucial issue of the era so it is deliberately included. The Gnomes as Jews things has to do with adapting the world views of the 19th Century and mating them to the setting rather than Troika wanting to make the comparison themselves out of a genuine belief that they hold.

Sadly, today, so few care for this differentiation now...

If you're seeking the former, then to put it one way, you need to find the racism version of "rape games" like the Rance series instead of bringing up games that contain rape as a story component and want to delve into its impact seriously, like Fallout NV did with that one NCR quest.

A properly Racist RPG would have to implement a heritability system. Like in Crusader Kings, but with trait modifiers for each ethnicity. For example, >80% of newborn Niggers would be either "Dull" or "Slow", while in Whites this would only be <20%. Similarly, ~20% of newborn Whites would be "Quick", while for Niggers it would be only ~2%. 0.0001% of Whites would be a "Genius", but for Niggers it would be 0.0000001%.

This would be Scientifically Accurate Racism, reflecting the fact that dozens of genes correlated with higher IQ have been conclusively identified, and we know these genes occur in Whites and East Asians at much higher rates than in Niggers. Conversely, gene mutations correlated with low IQ and other detrimental traits such as aggressiveness have also been identified, and we conclusively know that these gene mutations occur at a much higher rate in Niggers.

To express this in an RPG form, a game would have to span through generations. It would take millenia of in-game time to even finish a playthrough. On player character's death, you would be able to reincarnate as one of your descendants. Elaborate genocide mechanisms and eugenics systems would have to be implemented.

You can count on there being an unhealthy obsession with 'visible' diversity, though.

Which means any gay or bisexual character is given that godawful 'campy' attitude and any 'diversity' takes the form of dark skinned characters with the rare Asian thrown in here and there. If they're feeling bold they might throw in a hot headed Latina who'll either be a misunderstood criminal or a maid.
