I think it's a pretty weird design choice. It suggests that the game had a really big budget that Vavra felt he had to fill with all sorts of bells and whistles ("Fuck it, we have cash, let's add a gay romance subplot!")

If it was just some random nigger. They intentionally made him fit the "highly skilled immigrant doctor/healthcare specialist" narrative.
As a bonus the racist local population also hates him. We the player of course see what a great guy he is. You can be sure that there will scenes in the game, made to make the player feel sympathy for him because of unjustified attacks from some stupid white person.

I mean everyone knows that ranged combat is broken in ARPGs where enemies can't traverse the battlefield properly, look at skyrim where you can hide on rock and shoot enemies, similarily i could do that in KCD as enemies can't climb. Just don't exploit that, there is no easy fix for that beside giving npcs assassin creed tier climbing abilities on basically every mesh in the game, and player with free jump ability, not tied to any animation point, will still have advantage.

The Tomb Raider reboot slightly hinted of Lara engaging in some carpet-munching action with Samantha. (((Cuckmann)))'s teenage lesbo fantasy in TLOU: Left Behind was seen as something innocent and a product of Elie just being "alone and confused". All games by Faggotware had "completely optional" gay relationships which got more and more prominent and front centered. And look where are we now.
