Did you equip/unequip an item? Cause that obviously takes a turn and lets you die to damage over time effects like bleeding or burning.

That's probably what happened. I suppose that's better than a bug, but it's still silly that it happens during the pre-transition check to see if I want to keep or discard the loot I found, after I've already pressed the button to exit the level.
