I found the problem.

Intermediate hops can throttle their TTL exceeded messages or filter them out. Having losses on hop #5 when hop #6 has no losses means that traceroute/mtr output isn't reliable there.

Got a hangup at 5:44 am PST, didn't really check when it resolved but it did last for at least 3-5 minutes so I went and did other stuff, no 504 or anything like that just a long stall. First one in quite a few days.

Same, at 14.45 local time. Around 5 minutes. I thought the site was down. Also happened yesterday.

It happened on three different IPs.

Those were likely backups so that can't be helped, the database gets locked when it's being dumped so there will be a few minutes while you won't be able to access the site or forums while the backup is in progress.
