And it is conceivable that contemporaries know fuckall about proper ways of dealing with it. Knowledge has been lost previously. Read up on losses caused by scurvy to various navies of the Age of Sail. Fuckers kept discovering and forgetting the cure for 200 years, while loosing ginormous amounts of manpower.

So what distinguishes this from all those other interchangeable medieval fantasy games?

For one its not based on a derivative fantasy world or realm but its located in 14th century Europe. So its something akin to darklands. That means no femboy elves, no stunties, no pink haired orcs or other kinds of fantasy scum. This alone is massive incline. Another incline is that the gender of the protagonist is set as male. That means no retarded gender neutral dialogue for the player character that could be interchangeable with an object like most RPGs this days.

At the very least there are kobolds living deep in mines and some other monstrous people up in the hills, probably the ones that look like shadowrun trolls in concept art.

They didn't keep their holdings in eastern Europe for very long and when they did, they relied more on the local Tatars (the volga tatars in particular are a fairly white looking group), as the official government for where the portions of the empire did remain.

We don't know the full context as of yet.
And we likely never will, because it's a trailer, not a cutscene lifted from the middle of some quest. A trailer like this is supposed to show a short story. If there is supposed to be nuance, it should be in said trailer. And it's not like it's hard to craft the bad guy they needed either – all they had to do was replace the retarded goal of "killing all the sick" with the lord being here to collect the taxes. The sick peasants obviously can't give them to him, because if they do, they'll starve to death – they're too sick to tend to their farms, and if they give what they have stored, they'll perish. The nobleman then can say he doesn't give a shit – they owe him the tax by law and he's there to take them, and take them he will, even by force if necessary. This gives him a realistic impetus to start roughing up sick peasants, and isn't even cartoonishly evil – it is precisely because there's an epidemy going on that he cannot afford skipping the tax, as his income is down the drain since the sickness is in every village, while his expenses are skyrocketing (there typically were epidemy counter-measures taken by the authorities of varying efficiency, not to mention disposing of the dead bodies. Since everyone's getting sick and dying, it also causes a labor shortage, meaning wages go up). A very realistic and believable conflict with equally believable conclusion, serving the exact same purpose as the original, but without retardation and even with some nuance if one thinks about it.

Iam not talking about "competency" but cowardice from the incompetent or common sense of the competent.

Why would a noble who can easily order others to do his dirty work go among the sick and personally, up close, supervise killing of the diseased.
For the same reason they hunt animals up close, despite the risks there.

How many nobles perished in hunting accidents? Or because they were beset by bandits after taking a risky, ill-advised route? There's historical accounts of nobles poisoning themselves in the pursuit of a cure for minor afflictions.

I agree it would be stupid to depict the nobility as incompetent and psychotic across the board, though it'd be realistic for some to be portrayed in such a fashion.

Ultimately it's also a video game so there's an element of suspension of disbelief in the same fashion as with horror movies. You could pick apart any given chase scene in a horror movie, for example, though it's there for entertainment first and foremost so certain elements are played up.
Sure nobility hunted animals and led the charge in battle, they were not risk averse instead nobles often purse dangerous endeavors in search for glory and profit.

But in no universe is risking getting the disease is in any way similar to risking injury while hunting or combat.

Being exposed to deadly disease is something completely different to being injured in combat or during hunt. There is no glory or anything else worthwhile in getting it, as it was seen as punishment from God.

Its one thing if you are forced to interact with the ill, but I just cant see any reason why a noble who can easily order others to deal with it would decide to just go and walk among the diseased.

The trailer looked nice, but without gameplay (I wouldn't call those last 5 seconds as gameplay) I can't judge.

The one thing that confuses me, is how the vampires are the hunted, and not the hunters, considering their martial prowess. They seem incredibly strong and powerful, and the most important thing: organized.

Well, I just assumed they are hunted as monsters. Maybe you are correct, and it's the beginning of the vampire thing, and we get to witness the vampires take over the human world. With only one man able to stop the scourge. Johnny half-blood with Witcher skills.

The devs are supposedly sharing more lore on livestreams. I heard this:

The vampires grow numbers of fangs as they age, like tree rings. They pluck one and stick it inside a person’s heart to make a vampire. Silver is toxic to vampires. The protagonist is suffering from silver poisoning that causes him to become dawnwalker instead.

he's not hiding between peasants. also unlike negros he's not even uncommon in eastern europe.

nah. nomads were used for centuries in wars between eastern european rulers even before mongol invasion and long after them. often they also were blood relatives of local nobility or just accepted christianity and pledged loyalty.
their war tactics and equip also had huge influence on slavs, considering they were main enemies.
