And special mention at no. 4 would be Deus Ex, the original, at 101 hours. I think I'm on my third or fourth playthrough at this point, and I don't need to tell you why its one of the best games ever made. Right now I'm using what I consider to be the definitive and best mod for Deus Ex, GMDX. It just improves on the game in so many ways that no other mod has done before. Screw Revision, GMDX is where its at. It makes the AI an actual challenge in comparison to the original, it adds so many little quality of life things such as being able to stand on your tippy toes by holding down both q and e at the same time, general UI things like being able to drag and drop items more efficiently and allowing for double clicking of items to use them, rebalancing things like weapons and augs and making skills like swimming (now athletics) actually useful, more particle effects, more interactivity with the environment, there's things like perks in the skill trees to allow for even further customization, they've made it so every time someone dies the same ridiculous "AUGHHHHH!" sound doesn't play, but a variety of sounds can paly instead. There's an amazing hardcore mode that only has checkpoints, disables quicksaving and the menus don't pause time, and they've added in a stamina bar that completely changes up how you move about the world, so you cant just run everywhere to avoid guards or constantly throw stuff at people or hit them with batons, you have to take into account your actions carefully. The mod just adds a huge layer of complexity on an already complex and amazing game, and adds some amazing QoL fixes to go with it. It makes one of the greatest games of all time even better by fixing pretty much all of the original game's flaws. I could go on and on. Right now I'm holding off continuing to play it, because supposedly in the next update they're going to replace the vanilla hilarious shitty running animations with motion captured ones. This is such a small change, but by god I feel that it would just improve the feel of the game so much. However, theres a chance it might not even be possible to add in the animations, so it might not be coming any time soon, but still, it's nice to see what they have in store regardless.

Plain and simple GMDX is Deus Ex if the devs had 1 more year to work on the game. So dont play vanilla game first, why would you do that? Just play GMDX. The only question is which version V7 or V8. If youre a purist then go with 7 but if you dont mind some extra mechanics that werent intended by the devs then go with 8. Its the better version anyway with better AI and pretty much everything else.
Dont know about Revision but judging by the name it doesnt sound good... I dont know play it last.
Yeah they both have HDTP and NV except GMDX lack some HD character models for some technical reasons but you dont need them anyway because they suck.

GMDX does: gameplay, bug patching, audio design, level design, and to a lesser extent graphics. Simulation design too although that often comes under a bit everything, but not always. Also upgrades the user interface mostly with a focus on function (so gameplay), but not always.
Basically it does a bit of everything in varying degrees except plot and soundtrack. It was all a matter of context to me: improve what demanded work, leave what was perfect. The primary focus was indeed gameplay, but labeling it a gameplay mod doesn't do it justice since it does so much more.
